Two ways of completing your registration!

First way is to click the link above, download the registration form, fill it in, save it and email to the registration POC.  Don't forget to complete payment on Zeffy.  If you prefer send a check please send the check to the POC listed on the registration form.  Please do NOT send cash.  You can opt to send the completed registration form and payment by USPS.

Second way is to complete the online form below.  Once completed you will receive confirmation.  Again payment can be made by Zeffy or by USPS.  Please do NOT send cash.
